Have you attended prenatal classes in the past?

· Classes are taught at either our Sherwood Park or St. Albert locations. Please indicate which location works better for you (please note that location is dependant on the number of registrants, where the majority of registrants are coming form, etc.) We will contact you with the location and directions a few days prior to the class.

· All classes start at 10 a.m. and finish at 5:00 p.m. with a break for lunch. Please arrive a few minutes early so that class is not delayed.

· Please bring a potluck dish for sharing between 4 to 6 people. Doula Connections will provide beverages. A kitchen is available if something needs to be kept cold or heated in the microwave. Please indicate here if you or your partner have any food allergies

· The cost of your class is $100.00. Once you have submitted this form, you will be given your choice of payment options.
