Have you attended prenatal classes in the past?

· Classes are generally taught in St. Albert. Please note that location is dependant on the number of registrants. We will contact you with the location and directions a few days prior to the class.

· All classes start at 10 a.m. and finish at 5:00 p.m. with a break for lunch. Please arrive a few minutes early so that class is not delayed.

· Please bring a potluck dish for sharing between 4 to 6 people. Doula Connections will provide beverages. A kitchen is available if something needs to be kept cold or heated in the microwave. Please indicate here if you or your partner have any food allergies

· The cost of your class is $100.00. Once you have submitted this form, you will be given your choice of payment options.
